OPERATION KIDSIGHT
I thought it would be helpful to include recently asked questions.
Q. What if the information on the screening form isn’t completely filled out?
A. Information obtained on the screening form helps identify each child with certainty that the results information is passed on to the child’s parent or guardian. No identifying information collected is
shared with anyone else. It is important to know the age group we are screening. Information such as number of children screened and their age and the percentage of referrals, help determine the
benefit of this program.
Q. What is the age group for KidSight screening?
A. KidSight screening is for children between the ages of 6 months to 6 years. Occasionally, 1 or 2 of the children included in the screening exceeds the age of 6 and are included along with the
interpretation / results. It is not the equipment that defines the use, it’s the Operation KidSight Program
Q. Who notifies the parents of the screening results?
A. The results are mailed to the parent/guardian from the main office. A printout is mailed to the screening club chairperson. While the parent/guardian are required to sign a consent form, we still have to be cognizant of our responsibility to be compliant with the HEPA laws. As we speak, the legal department is reviewing the forms.
Q. How do I make my donations?
A. Club Donations to OKS should be made to the Health Services Board – OKS.
Outside/Individual Donations can be made to Lions Sight & Hearing Foundation, Inc. – OKS
(a 501c3) and used as a deduction on Income Tax.
Equipment Usage Donations should accompany screening cover sheet.
